If the cause is sludge, yes, and cold at the bottom with a hot top is the classic sign. If the radiator is cold at the top instead, it usually just needs bleeding. Trapped air, a failed valve or a seized pump are different faults, so we check before recommending a flush.
Only when it needs it, which for a well maintained system with inhibitor and a magnetic filter can be ten years or more. The filter is the giveaway. If it is packed with black sludge at every service, the system is telling you it needs cleaning.
